Repeat prescriptions are issued for established, stable conditions where your GP has put a plan in place. Use the form below to submit your request.
Please allow 48 hours for your prescription to be processed and sent to your pharmacy. Do not wait until you have run out of medication — request at least 2–3 days before you need it.
Before You Request
- Repeat prescriptions are for medications already agreed with your GP
- If it has been more than 6 months since your last consultation, a review appointment may be required first
- Medications requiring blood test monitoring or blood pressure checks may need a review before reissue
- New symptoms or changes to your condition require a GP consultation — do not use this form for new issues
Prescription Fees
- GMS / Medical card patients: prescriptions issued free of charge
- GP Visit Card patients: free where covered under your card
- Private patients: a repeat prescription fee applies and must be paid before processing
- Free contraception scheme: eligible women aged 17–35 can request contraception prescriptions free of charge
Please note: Never use this form for urgent medical needs. If you are unwell, please call us or book an appointment.
